Prepare for Your Visit

Recommend the Virus Testing if:

  • You were exposed to someone with Covid-19

  • You were exposed to someone with symptoms consistent to Covid-19, but did not get tested 

  • You are concerned you may have been exposed and want to make sure you are not carrying the virus and are asymptotic 

  • You have symptoms consistent with Covid-19


Recommend the Antibody Testing if:

  • You were sick with symptoms consistent to Covid-19, and want to know if you had it

  • You want to know if you have antibodies that may protect you for future risk

  • You want to return to work and want to know if you have had the virus



Form to complete before the appointment

In order to fill out the form during your visit, You will need:

  • A phone with texting and photo capability​

  • A state issued ID (drivers license or other legal form of identification) 

  • Medical insurance card 

  • If you do not have an insurance card, you will need to have your insurance ID number, group number, policy holder’s information, phone number and address for claims.

  • Self pay ($95 visit plus lab fees charged via credit card only) is available if you are uninsured, or you wish to have testing in the event we do not accept your health insurance. 

  • Insuraces that are not accepted:​

    • Kaiser​

    • Centura

    • Blue Cross Pathways (different than blue cross blue shield) 

    • Denver Medicaid (different than normal medicaid)


Instructions once you arrive to the testing site

  1. Follow the signs for testing entrance. From Grant St, turn onto N. Suburban Drive and follow the signs. 

  2. Do not leave your car and keep your windows rolled up. 

  3. Follow all sign instructions. You will need to have a phone with texting and photo ability. ​

  4. Do not exit the line until you are motioned to move your vehicle forward. 

  5. When it is your turn to test, one of our providers will explain the testing procedure, obtain your sample and may ask additional questions or perform additional assessment as needed. 

  6. After your test has been collected, you are free to go home. 


Drive-Up Testing Site Information

  • Drive-up testing is available in the parking lot of our Thornton location at 9195 Grant St. Thornton, CO 80229.  


  • Testing is available Monday through Friday, 8:00 a.m. through 1:00 p.m.

  • Make an appointment online.

  • Appointments are recommended. While we can accept drive-ups without appointments, supplies are limited and will be reserved for patients with appointments. 


  • All persons within the vehicle, even if they are not being tested, will need to wear a mask or face covering

  • Safely serving the community is priority number one. If, for any reason, we are not able to safely administer tests, testing will be delayed until we are able to keep our patients and staff safe and healthy.
